1099-R tax forms have been mailed to the addresses we have on record for retirees and beneficiaries. This form contains essential tax information including reportable income, taxable income, federal taxes withheld, and California state taxes withheld, if applicable.

Follow these steps to view and print your 1099-R online or use the 1099R Quick Reference Guide:

1. Log in to MySamCERA (www.mysamcera.gov) on a computer or laptop
2. Click on Your Plan on the left navigation menu
3. Select 1099R Tax Information
4. Select a 1099R from the list to view and print
